I was wondering what igM positive meant?

Two years ago i was treated for h pylori with two rounds of antibiotics one was one week, and another one was two weeks. The first and second line treaments were different so im sure i completely eradicated the bacteria, i had two stool tests the following months tested negative and felt a little better and worse better and worse depending on my diet. Two years later i still deal with gastritis but had breath tests,stool tests and igG testing negative recently too.

However my igM is positive. I was wondering what is the clinical significance? Can I still be elevated after two years of eradication? or Do I have the bacteria again? I have suffered maltose intolerance ever since I eradicated H pylori and everytime I take PPIs to heal my gastritis, my reflux gets worse and worse, so I dont take them and dont even touch H2 blockers anymore, they only work temporary but my reflux gets better when I stop taking antaacids but unfortunately my gastritis hasnt healed ever since i eradicated h pylori because i could not take ppis after eradication anymore. Currently controlling my gastritis with diet only and Im wondering if I should go on round 3 of antibiotics just because igM is positive?

Any suggestions on what should i do? Even if the endoscopy i get (maybe in the next few weeks) is negative, along with everything negative, should i still try a 3rd line treatment (assuming the first two failed)?

During an early infection, your body produce IgM to fight off the infection. In the 3rd or 4th day of infection, your body will stop producing IgM and start producing the more specific IgG and IgA. So, IgM usually is used to tell doctor how long have you been infected.

IgM is less important for H. pylori because H. pylori is a chronic infection. Usually people measure IgG or IgA for H. pylori infection. Not too sure the reason for IgM detection.

I think you should get your breath test done and then decide if you should get another round of treatment.

HI I just had a stool test and turns out im h pylori positive, the past weeks I have had more stomach burning and more symptoms than my usual gastritis symptoms but overtime my foods have become much more limited.


Ive been basically living off brown rice,potatoes,carrots and egg whites since february. I had a suspicion i had h pylori because certain foods gave me symptoms as if they triggered an inmune reaction such as white rice basically all overcooked starch. I found out i have become more and more intolerant to maltose since this year hence i cant any overcooked starch(contains maltose) which has limited my foods to steamed vegetables and very undercooked brown rice.....i felt an instant reaction when eating overcooked starch or maltose but no problems with all other types of sugar.

However i have had symptoms getting worse over time since beginning of feb 2015 now its a year later and i cant touch so many foods compared to feb 2015 where i could tolerate starches.

I dont know if its a reinfection or if its the same h pylori i didnt get rid of in 2014 (had stool test confirming i wiped it out) but i moved to another country and could not follow the same bland diet(although still eating bland) and ever since i been getting worse little by little.

These are the first two treatment therapies i had in the US:

1. amox,clar,nexium x2 20mg i think.
2. tetra,metro, nexium x2 20mg .

The first one did not fail i did not comply bc i was hospitalized for other reasons and the doctors discontinued my treatment. after i got out of the hospital my gi told me to take 2nd treatment to make sure i wipe it out since my treatment (first) was stopped mid way.

so Now i dont know if i should do the first two treatments again or a 3rd line treatment assuming its the same bacteria that(a grew over time since last year(i have gotten worse and worse little by little so i have a deep suspicion its not a reinfection). I have so much reflux now its unbeliavable and extremely dangerous.

Unfortunately PPIs give me more reflux and my reflux gets better when im completely cleared off PPIs AND my gastritis over control. SO I can't really risk being on first and 2nd line treatments too long because PPIs destroy me. In fact last year aroudn these months i went to another doctor who gave me nexium for gastritis (no treament for h pylori since breath test was negative) but it destroyed me....after i took nexium i got an stomach infection, broken bones, and all other stuff so i stopped it cold turkey and ever since I have had insane reflux which got better after i stopped nexium for a few months and cleared completely after 3 months off PPIs but then the cycle starts i take acid reducers for gastritis, reflux gets worse, i stop ppis reflux gets better gastritis worse etc....

Can you please suggest a good third line treatment with high efficacy rate? that way i can avoid taking PPIs for too long, they can literally kill me(the reflux gets so bad after taking them for too long, i throw up blood and im still recovering from the last time i used Acid reducers which was a few months ago).

My IgG is negative, igM positive and stool test positive.

Could you please suggest a third line treatment or what treatment i should follow? Doctors here are clueless i live in a third world country now and they dont even treat h pylori, almost most of them wanted to give me PPIs to control the symptoms and not get rid or test for bacteria or do any other tests despite my severe symptoms and being very underweight. I basically had to self-prescribe these blood/stool tests to prove im infected.

I will take your suggestion to my doctor. I made an appointment with one who knows about h pylori. thank you so much.

Thanks for sharing your story. The best way to treat H. pylori is to get antibiotic sensitivity test done. With the knowledge of what antibiotics your HP is sensitive to, you'll have a good cure rate. Unfortunately not many doctors have access to such service. So, you'll have to find a good specialist that is able to do that. otherwise, you'll just have to keep trying different antibiotic treatments, take the risk of not making other gut bacteria multi drug resistant.

On the other hand, have you speak to any dietitian or immunologist to see if you are allergic to certain food?
