H. pylori, antibiotic and endonscopy

Post by yin »

Hi I'm new on this forum. The information posted here is excellent and has been incredibly helpful.

I was diagnosed with the H.pylori infection about 12 years ago and at the time treated with antibiotics (Can't remember the name, think it was prevpack) for 10 days. After that I felt fine and there seemed no need for follow up.

About 7 months after the first treatment of antibiotics, my heartburn symptom came back. I visited a GI who suggested I just try and live with the symptoms as the antibiotic treatment could be worse for me than the bacteria itself. I took that advice and used probiotic and digestive enzyme whenever I experienced symptoms and things were manageable until the spring of 2013. At that time I started to become sensitive to nuts, and created a lots gas,I then visited another GI doctor, did a blood test which showed H.pylori as positive. The doctor then ordered an endoscopy and colonoscopy tests, and the test results were: Focally active chronic gastritis in antrum and mild chronic gastritis stomach body, also H.pylori positive.

I felt much better after the endonscopy and experienced no further symptoms. Occasionally I did require ppi to control my heartburn, otherwise everything was fine. In 2014 I ate some spicy food and my heartburn and gas became a more serious problem. I was still able to manage the symptoms with ppi and other natural/alternative means, but by summer 2015 I had enough and started my second antibiotics treatment (omeprazole 20 mg,metronidazole 500mg,clarithromycin 500mg) for 10 days.

Four weeks later, I had a breathe test which showed I was H. pylori positive. My symptoms were continuously getting worse after the treatment, and I'm not able to go a day without ppi, nor eat large meals, meat or dairy products. One week ago, I started my third round of treatment (omeprazole40mg, tetracycline 500mg,meronidazole500mg,bismuth). I started feeling a little better for the first three days, but then things got worse again and I feel as though this treatment will fail again. I then spoke with my GI doctor PA and asked that we perform another endonscopy test.

I do not believe any of this is normal, and the ineffectiveness of the antibiotic treatments has me deeply concerned. The GI doctor office called back today and wants to do the endoscopy this Friday. My antiobiotic treatment will end next Tuesday, so will this endoscopy test have any value? Can the doctor secure an accurate diagnose in the middle of the antibiotic treatment? The PA told me they want to check my gastritis situation and will order another breath test 4 weeks after the treatment. I need to decide if I should take the endoscopy this Friday. Your expertise advise will be greatly appreicated sincerely thank you.

your current tetracycline based quadruple therapy is a strong one. So, i would suggest you try finish the course and wait for a month before your follow up test. I think an endoscopy then would be more meaningful as you can check if the H. pylori is still present.

Thanks for your response - appreciated.

Would the endoscopy still be helpful to determine other potential issues, such as gastritis, intestinal metaplastic, or preneoplastic leasion before the antibiotic treatment is complete?

Could the endoscopy allow the doctor to determine what strain of h.pylori I have so that if this round of treatment fails that they could better target the next round of treatment? Or would that not be possible to detect given I am currently under treatment?

Thanks again

I had my endoscopy this morning since I was concerned that the current quadruple regimen has not improved my situation, also I did't want to wait for another two month, even though I'm in the middle of the treatment. These are the findings of today procedures (the pathology biopsy report will come out in about 7 days)
1) normal esophagus
2) no hiatus hernia
3) Mild gastritis of the entire stomach. a new 25 mm erosion in the great curvature of the body.
4) the antrum and 25mm erosion were biopsied.

The doctor prescribed me tarafate to treat the erosion. I should start carafate after the antibiotics treatment is complete. Here are some of my concerns;

1) It seems that the two round antibiotics treatment made the situation worse, I only have mild antrum gastritis before the antibiotics treatments. Does taking antioiotics secrete more acid in the stomach and the omeprazo 40mg twice a day is not strong enough to control the acid?

2) I really worry that the biopsy come out with some cell precancerous change. If it is true how can I do to stop the change?

3)Is carafate a best option for ulcer healing? I'm taking so many prescriptions now and its actually getting worse. Can I use aloe vera juice, gabbage juice and bismuth and probiotics instead of carafate?

4) Should I take my qutruple regimen with Lacroferrin, Vitamin B-complex and some plants oils(omega 3 and omega 6). I read from pubmed that it will help improve HP eradication rate slightly?

5) Can ulcer be healed by carafate or ppi without antibiotics?

Your advice is greatly appreciated.

it's not normal for ulcer patients to get low grade fever. but nauseous and vomitting are symptoms of H. pylori. You need to watch out for c. difficile related diarrhea.
