I got the tests results from my stool test... I still have H. Pylori in my system, which means that the first treatment didn't work. Recently, my stomach aches and nausea has gotten worse as well. Any recommendations? I called my GI doctor but he is on vacation. He should call me soon and probably prescribe another round of antibiotics. What are your recommendations as far as antibiotics go for a second treatment? I did lansoprazole, amoxicillin, and clarithromycin for my first triple therapy.

you may try PPI + Amox + Met + pepto bismol.

By the way, which country you from? ask your doctor about the local antibiotic resistance profile.

More information:

I called my GI doctor and he asked me to do another stool test to make sure the first one was not a false positive. Considering that my symptoms have gotten worse and that the first test came back positive, I told him "no" and I moved onto a different doctor... He was very flaky.

The new doctor put me on quadruple therapy: PPI-Bismuth-Met-Tetracycline. I have to take this for 10 days. I started on Thursday and on that day, the symptoms practically disappeared. On Friday symptoms got worse, especially in the afternoon. The major symptom since I've started antibiotics has been persistent mild nausea, with very short bursts (5-10 seconds) of intense nausea, on the verge of vomiting (never vomited). Today Sunday I have burning pain in my stomach and mild nausea as well... This whole thing is extremely frustrating.

I'm originally from Spain and I have read that Spain is one of the countries where H. Pylori has the most resistance to clarithromycin. However, I live in the US. Also, it probably didn't help that I had to take azithromycin for an STD I had in February (so I'm making the assumption that my H. Pylori got stronger).

I should finish the antibiotics course on Saturday.

it is true that your Feb treatment for STD had probably made your H. pylori stronger. Unfortunately we dont know if you had H. pylori then. Did you do a test before this quad therapy?

you are probably experiencing some side effect. try to finish the antibiotics if possible. Unless the side effect is really untolerable. Discuss with your doctor about this also, just in case you need to be hospitalized.

Everything started in February, when I had to take ceftriaxone, azithromycin, and a week of doxycycline for the STD. That solved the problem.

But a week after finishing treatment, I started developing the GI symptoms. So I was diagnosed with H. Pylori and then was given triple therapy for 14 days (lansoprazole, amoxicillin, and clarithromycin). That helped for a month or so after treatment, but then the symptoms came back stronger. Then I took a stool test six weeks after the first treatment and it came back positive for H. Pylori still.

So now I've been given quadruple therapy (omeprazole-Bismuth-Met-Tetracycline) for 10 days. The symptoms are still there, but I feel that now I have a mixture of gastritis symptoms and side effects. They are annoying but bearable, so I'm totally planning on finishing the antibiotics course.

My question is: assuming that the bacteria has been in my stomach for a long time, do you think that since I had to take doxycycline in February the bacteria might've gained resistance to tetracycline?

good question. but, knowing that tetracycline targets one of the most conserve gene, the 30s rRNA. a minute mutation in that gene will cause cell death. So it will be difficult for H. pylori to become resistance to tetracycline. We have not seen a real tetracycline resistance case so far. Nevertheless, nothing is impossible.
