Triple Therapy How Best to take

Hi, I’m currently on a triple therapy of Pantaprozole amoxicillin & metronidazole. I initially had Omeprazole Clarithromycin and amoxicillin but reacted badly (rash, shortness of breath and extreme fatigue) to Omeprazole and I think also Clarithromycin. Changing to the current 3 the treatment has been much easier.

I did however have a rough couple of days and my stomach was unhappy to say the least. I separated out taking the drugs for an hour to see what I was reacting to and check the new antibiotic. I found this much easier on my stomach. So wondering if it is important to take the amoxicillin and metronidazole at exacly the same time, or if it’s okay to continue separating them out a bit to ease the side effects?

it's ok to spread out. but it's more important to take them consistently at the same time. That will keep the antibiotic level constant in the blood.

That’s a relief to hear thanks. I am taking them consistently. When I was given Clarithromycin I was advised to take them twice a day 500mg each but the Metronidazole has been advised three times a day at 400mg a dose. It seems a very high dosage, do you know why there is a difference or if three times is necessary?

we give the same dose.

Clarithromycin 500mg, 2x a day
Metronidazole 400mg, 3x a day

In the laboratory, metronidazole is one of the easiest drugs that H. pylori can become resistant to very quickly. So a higher dose will prevent that.

Okay that makes sense. Thanks so much for providing this resource I’ve found reading the forum and now interacting to be so helpful. Its not an easy treatment to take, I’ve been on the verge of stopping, and coming here has helped me continue.

These meds are very different to a standard course of antibiotics, I think compliance would be much higher if patients were offered some form of guidance/support line here in the UK.

I will do. I’m actually taking the h-pylori eradication on the advisement of my Consultant. I have chronic T cell lymphocytosis, have done for nearly 20 years. After numerous tests my Consultant believes it’s reactive. Separately, severe gastritis started in October 2017 after an infection that caused severe localised pains all over my rib cage, mouth ulcers and later I believe stomach ulcers and then reflux. Also sensitivity to most foods developed and I had to go gluten and most dairy free.

Interestingy my Consultant said he had one other patient with a similar haematological picture whose gastric symptoms and blood count resolved with a course of eradication therapy for h-pylori! So I’m really curious to see if this treatment resolves that for me too. I’m also IGA deficient, so I believe my immune system will have had a hard job fighting bacteria in areas like my stomach and the antibiotics will hopefully be beneficial.

I was also exposed daily to carbon monoxide poisoning prior to the infection for around 8 months and think the gastritis built up over this time with a lot of inflammation.

So my stomach has been through a lot! Hopefully the eradication therapy will start me on a road to recovery.

Interesting. that will be another evidence that H. pylori can have impact on other organs other than stomach.
