PPI is slow acting but a more complete acid blocker throughout the dy. H2 blocker is a fast acting, but doesnt last very long. If you think you can time your body clock well, as in you know when your acid will come in, usually around meal time, then you can use H2 blocker to control it. if you want a more complete blockage of acid, so you dont have to worry about when the acid would come, then you can take PPI.

People take PPI for years. So it's a pretty safe drug. However, when you stop PPI, you have to stop gradually. Otherwise your acid secretion will suddenly rise and you'll get reflux.

I am not so bad with the reflux but I think the increased acids after HP eradication cause my stomach to heal very slow if it heals at all. I might try with 1x20mg PPI but I have to decide how much time to do it and then when to stop. Or 1x40mg famotidine... As I am not so bad even without acid suppress drug probably small amount will help...

Do you think there is any reason to take also bismuth salt, I am taking from 3 days liquid pepto bismol (3x15 ml/790 mg per day) but still does not feel any dramatic difference?

bismuth is antibacterial. so it will not help with your acid. taking PPI or H2 blocker is to prevent burn on your throat. So, it is better to take either one if you can.

Actually burning in my throat is not the biggest concern as it's not so strong - if it's only that is not a concern. I just have elevated acids inside the stomach which cause extra burping, feeling of burn and pinching, etc inside the stomach especially when empty. Also my hypotheses it is preventing healing the gastritis. For the PPI I have mixed experience with esomeprasole (nexium and other cheaper brand). Which will be the best PPI like long lasting action?

you can try rabeprazole. we like rabeprazole because it is metabolised differently compare to other PPIs. so it stay in the body longer.

I am taking rabeprasole 20mg 1x per day (morning) almost 2 weeks now. At the beginning didn't notice any difference (even first pill was awful as I think it completely stopped the acids for sometime). Anyway after few days I feel better.Generally I feel better now - can eat more things, less burps, less feeling nauseous after eating, and of course less acids and reflux symptoms. However my tongue is still white with not much visible difference than before, and I don't feel my stomach is resolved.
I've been in the same thing before, when taking esomeprasole - while I take it I feel better, although not completely ok, when I stop it after 2 days the acids are back, after 1w I start also feeling bad after eating. Not sure to what extend this is caused by reflux itself or by the gastritis, as I feel exactly the same like when they do endoscopy and they see the active gastritis... So I do not beleive the gastritis is gone and it's just esophagitis causing the symptoms.

So I am wondering now how much time to take the rabeprasole. Stop it and see how it will be or continue.

If you feel better with rabeprazole, you should continue on until you are absolute that you can stop. When you decided to stop, you can try alternate day rabeprazole or take half a tablet daily.

If unsure, you should speak to your doctor as he can examine your condition physically.

the thing is how to be absolutely sure when to stop. My indication is the white tongue which is still white. If it clears, then I can continue 1 more week and stop it, I guess. Maybe I should discuss it with a doctor although their opinion also vary how exactly to take the pills. For example there are doctors which use high PPI dosages freely and some try to use the minimum amount.

I think you need to judge yourself. Only you yourself know how much you need to take. If you feel that taking more makes you better, then take more.

I've been to my first doctor today showing him all the docs I have and he was impressed with all my self steps. :) Anyway he thinks that my symptoms are more to the range of reflux, and probably not coming from the gastritis. As all my HP tests are negative he told me to forget about hp for now. As he does not see now alarming reflux symptoms in the esophagus area, he advised to try step down approach with the PPI and to try gradually stop them. According to him this should be led by me and my feel. Also I can replace them first with H2 blockers and of course taking on demand PPI/H2/antacids when needed. Also his advise is to try stop eating on the place I lunch on the workdays for sometime (I already have some suspicion about the food there), and be careful what I eat and take some probiotics for a month.

To be honest I am not convinced that my main problems are from the reflux but I am not a doctor. I told him that I have bad feeling inside stomach like it's more to a gastritis, but he does not agree. In my opinion reflux effect is more to the bad feeling and pain in the throat but not making you sick and feeling that your stomach is not working. And indeed last few months I am most of the time on some antisecretory drugs (mostly H2) and maybe it's time to begin teaching my stomach to work on it's own if the bacteria is dead. Maybe, I will see... The journey continues. I am not very optimistic anymore so I will see. Hopefully it will resolve with time.
